STREET OUTREACH PROGRAM MANAGER (Click here for job description)

For Participants of LPCS Street Outreach Services to reach their maximum potential, each needs to be engaged in a stable, safe, respectful, and trusting relationship to support and motivate them to make life changes. The Street Outreach Program Manager is responsible for leading a team of Outreach Workers to identify individuals experiencing street-based homelessness in the City of Chicago and establishing and maintaining safe, trusting, and respectful relationships with them. Outreach service participants will become more invested in their own life changes when they feel connected to something larger than themselves – a community. The Program Manager encourages Participants to engage in LPCS’ community and in their immediate community. Through this relationship, the Program Manager connects Outreach service Participants to other social service agencies, resulting in greater self-determination and stability.

ICLP - CASE MANAGER (Click here for job description)

In order for Independent Community Living Program (ICLP) residents and Lincoln Park Community Services (LPCS) clients to reach their maximum potential, each need a stable, safe, healthy, positive place in which to make life changes. The Case Manager is responsible for establishing and maintaining a safe, healthy and respectful relationship with residents of the ICLP, guests of the Interim Housing (IH) program, and clients of LPCS living elsewhere in the community. Clients will become more invested in their own life changes when they feel connected to something larger than themselves – a community. The Case Manager encourages clients to engage in LPCS’ community and in their immediate community. Through this relationship, the Case Manager connects clients to other social service agencies, resulting in greater social skills and more positive decision-making abilities.

PROGRAM DIRECTOR (Click here for job description)

In order for LPCS programs to reach their maximum potential, guests need a stable, safe, healthy, positive place in which to make life changes. The Program Director is responsible for creating and ensuring a safe, healthy, and respectful community for Interim Housing guests, community clients, and permanent supportive housing clients by developing innovative programs and providing leadership to staff. The Program Director works closely with program staff and the CEO to design, fund, implement, and evaluate all programs.

For LPCS clients to reach their maximum potential, each needs a stable, safe, healthy, positive place in which to make life changes. The Case Manager is responsible for establishing and maintaining a safe, healthy and respectful relationship with Guests and Graduates of the interim housing facility and clients of LPCS living elsewhere in the community. Clients will become more invested in their own life changes when they feel connected to something larger than themselves – a community.  The Case Manager encourages clients to engage in LPCS’ community and in their immediate community. Through this relationship, the Case Manager connects clients to other social service agencies, resulting in greater social skills and more positive decision-making abilities.

For Guests of the LPCS Drop-in Center to reach their maximum potential, each needs a stable, safe, healthy, positive place in which to make life changes. The Drop-in Center Program Manager is responsible for establishing and maintaining a safe, healthy, and respectful relationship with Guests of the Drop-in Center. Guests will become more invested in their own life changes when they feel connected to something larger than themselves – a community. The Program Manager encourages Guests to engage in LPCS’ community and in their immediate community. Through this relationship, the Program Manager connects Guests to other social service agencies, resulting in greater social skills and more positive decision-making abilities.

In order for LPCS programs to reach their maximum potential, Guests need a stable, safe, healthy, and positive place in which to make life changes. The Interim Housing Coordinators are responsible for creating and ensuring a safe, healthy, and respectful community for Guests. Guests will become more invested in their own life changes when they feel connected to something larger than themselves – a community. They will see how their actions affect the community, and how they can learn and benefit from others in the community. Creating this sense of community will result in greater social skills and more positive decision-making abilities, which in turn will lead to more lasting life changes.
